OrthoRewards Terms and Conditions
					
Enrollment, membership and all related benefits of the OrthoRewards Program (the "Program") are offered at the discretion of Ortho Technology, Inc. ("Ortho Technology"). By enrolling or using the Program or collecting OrthoRewards points, you agree that you have read and understand these Terms and Conditions and are bound by all of them, as changed from time to time by Ortho Technology. These Terms and Conditions supersede and replace all previous terms and conditions and are subject to change from time to time at the sole and absolute discretion of Ortho Technology.

Points
- Points will accumulate on the basis of ten (10) points per U.S. $500.00 of total purchases during a one-month period.
- Points are distributed into members' OrthoRewards online account at the end of each month.
- In order to receive points, your product account must be current (not aged more than 60 days) with Ortho Technology.
- Ortho Technology reserves the right to subtract points from members' OrthoRewards account to make adjustments for returned products. Ortho Technology reserves the right to audit any and all member accounts at any time and without notice. Ortho Technology is the final authority with respect to all matters involving a member's OrthoRewards account.
- Points are not assignable, saleable, transferable, replaceable or redeemable for cash. 
- Points cannot be pooled with other program participants. 
- If any member has any questions regarding point activity, then adjustments shall be considered by Ortho Technology only if Ortho Technology receives the member's written inquiry within 90 days from the date of the purchase or activity in question. Ortho Technology is not responsible for the delivery of incoming correspondence, and verbal notification is not accepted. All adjustments shall be at the sole and absolute discretion of Ortho Technology. 
- Members' points will remain in members' account for two calendar years. If not redeemed within this two year timeframe, points will expire January 1 of each year. Expired points will not be reinstated. 
- Ortho Technology, its agents, and employees will not be responsible for award requests or correspondence lost or delayed by any postal service or carrier. 
- Ortho Technology reserves the right to add, modify, or delete points terms, conditions, benefits and/or award levels with or without notice, as determined in Ortho Technology's sole and absolute discretion.
- Ortho Technology reserves the right to terminate the OrthoRewards Program without advanced notice, and the right to determine the forum, content and manner for giving of such notice, all in Ortho Technology's sole and absolute discretion. No compensation shall be owed to any member should Ortho Technology terminate the Program.
- The OrthoRewards Program is void where prohibited by law.

Rewards-General 
- Determination and payment of tax liability, fees and charges is the responsibility of the program participant. 
- Members can accumulate up to a maximum of 5,000 points at any one time. No points shall accumulate in excess of such amount. Ortho Technology reserves the right to impose a mandatory year end redemption period and terms and conditions applicable thereto, in its sole discretion.
- An OrthoRewards member may use a maximum of 5,000 earned points to procure awards during a calendar year. Any additional points (not exceeding the maximum points that can be accumulated) shall be suspended for use until the following calendar year. 
- Ortho Technology shall not be responsible for lost or stolen OrthoRewards points, gifts and, gift certificates. 
- All points are subject to revision, modification and termination by Ortho Technology, in its sole and absolute discretion. No compensation shall be owed to any member on account of any such revision, modification or termination by Ortho Technology.
- All rewards will be issued by Ortho Technology. Redemptions may be obtained by calling 1-800-999-3161, or by ordering on-line at: www.orthotechnology.com.
- All gift certificates listed are from independent suppliers with the exception of Ortho Technology Supply Rebates, and are subject to the terms and conditions set by each merchant. Each independent supplier reserves the right to change, suspend or terminate any or all rules, regulations or offers at any time without notice. Ortho Technology shall not be liable for any supplier change(s), and may add and delete participating merchants from time to time in Ortho Technology's sole and absolute discretion. 
- Ortho Technology makes no guarantees, warranties or representations of any kind, express or implied, with respect to any such merchant items, and shall not be liable for any loss, expense, accident or inconvenience that may arise in connection with the use of such items. 
- Certificates are listed in U.S. currency unless noted otherwise on their face. 
- Ortho Technology reserves the right to discontinue awards and the Program at any time, to be determined in Ortho Technology's sole and absolute discretion.
